Below are links to the required tax and identity verification forms. Please download and fill out the forms. You may either email a digital copy to Human Resources or print the forms out and bring them with you on your first day.

Instructions for forms:

1. Click on each link below to download the required forms (All 4 forms are needed)

   3. MI W4

   4. Authorization for Direct Deposit
      

2. Either:

   1. Fill out and sign each form using a PDF editor.

   2. Print each form, fill out and sign with black ink.
      

3. Return the completed forms by one of the following methods.

   1. Email all forms as well as a copy of your License and Social Security Card to Xtreme Clean Human Resources.

   2. Bring the completed printed forms with you on your first day to give to the market manager.
